Día Health serves the Texas community at several locations in the San Antonio area.
Send a request to be part of our practice by clicking the link below and choosing Pre-Register in the bottom right hand corner of the page.
Existing patients can follow the same link to access Patient Portal or download the Healow smart phone application where they can request appointments, medication refills, and communicate directly to the practice.
